New patients are required to pre-register online prior to scheduling an appointment. Once completed, one of our staff will call you to schedule. The following will be required on the day of your appointment.
- Please bring your insurance, pharmacy card and a valid photo ID.
- You have the option of printing the forms below and bringing them with you or filling them out at the time of your appointment. All medical records pertaining to the reason of your visit.
- An up-to-date medication list with the specific dosages you are taking (including a list of medications you have already tried to reduce duplication of prior treatments)
Please take the following into account.
If your insurance requires a referral, please make sure your referring doctor has sent it to us prior to your arrival. If a referral is not on file, we will not be able to complete the visit as services will not be covered by your insurance.
Our office provides courtesy insurance verification to understand your coverage and benefits, but it is patients' responsibility to make sure that we're in-network with your insurance carrier. We will let you know of any out-of-pocket expenses due for your visit.
Any out-of-pocket expense is due at the time of the visit. No checks will be accepted until care has been established.
